Registration Requirements:
1. Payment: A minimum initial deposit of $10 for each sport is due at registration time in order for your registration to be valid. Register by the notified deadlines on the home page to guarantee your child's spot on the team for the season sports. The balance of the registration fee will be due before team practices begins. If you have trouble completing your registration, please contact Lynn Flowers

2. Valid Physical:
All athletes participating in athletics must have a current physical examination record on file in the Parish/School office. (Current is defined as on or after April 15th for the following School year).  If the physical is not  turned into GA Athletics, the child(ren) will not be able to participate in tryouts or practices until it is turned in. No exceptions.

3. CYO Assumption of Risk:  Parents must agree to the Assumption of Risk during the registration process.  The registration will not be process unless the "I agree" is checked off.  Hence by completing the registration you have met this requirement.

4.  Concussion Awareness Form:  Parents and athletes must read and sign the Concussion Awareness Form found in the Documents & Forms link in the left menu.

5.  Catholic Parish and Catholic Religious Education Verification:  If you are a member of Guardian Angels Parish and/or school, the verification is done for you. In this case, there is no paperwork that is required. If you are not a member of Guardian Angels parish and/or School,the required A-form is to be signed by your parish's pastor and Director of Relgious Education (DRE) and turned into GA Athletics. A copy of this form can be obtained from the GA Athletic Director.
